There is a video on the Matrix website for these types of issues.
My suggestion is that if you are in a section where you are filling out individual questions, you need to click on Mastered. The only option that I know of for Surpassed is when you respond to the initial questions, where you can surpass a whole section A or B based upon your response.
For the spoken word, I believe that the reference is to using their natural voice. However, if you prefer to mark Spoken Word, add the clarification in the notes section (Spoken with Device). The important part is your consistency.
